Trevor Reznik is an industrial lathe operator who has been suffering from a severe case of insomnia, having not slept in an entire year. As his physical and mental health rapidly deteriorates, his emaciated body and frayed sanity begin to spiral out of control. After causing an on-the-job accident that severely injures a coworker, Trevor finds himself targeted by paranoia, mysterious cryptic notes left in his apartment, and a sinister recurring colleague who nobody else at the factory claims exists.
